Team-Based Approach Improves Diabetes Care: ACP Diabetes Initiative Extended for Two More Years

The American College of Physicians invites your practice to participate in Closing the Gap: Diabetes Care (CTG), a team-oriented, multi-format, practice based, on-line educational intervention. CTG will help physicians and their staff to develop and improve strategies for creating systems change in their practices, to improve the care they provide to patients with diabetes.
This program incorporates the Chronic Care Model for systems change, the Institute of Healthcare Improvement (IHI) Breakthrough Series Model of social learning, the Plan-Do-Study-Act (PDSA) cycle for quality improvement.
What are the benefits?
- Free participation. There are no tuition or registration fees
- Free CME credits accepted by the ABIM for Part 4 Maintenance of Certification (MOC) and NCQA certification
- Educational materials, chart tools, patient tools, data abstraction forms and support materials
- Access to ACP’s most eminent faculty who will give you hands on advice and coaching throughout the program
- Assistance with IRB approval. ACP is responsible for overall IRB approval of this project and we will provide you with all IRB approved materials, consent forms, etc and we will assist you with any IRB applications you may have.
- Reports on your practice measurements
- CME credits (for the educational material on the web) for physicians
- ABIM recognition of this program for re-certification
- CE credit for nurses and other allied health professionals
What is involved?
- Designate a team of 1 physician, 1 nurse or allied health professional, and 1 administrator:
- Provide practice data from 35 patients via chart review. Baseline prior to start of the program. The second measurement is taken after completing the training videos and educational materials, approximately 4-6 months after completion of the baseline data.
- Send two progress reports for review
- Obtain any IRB approvals necessary from your institutions, ACP will assist you
What are the risks?
Physician chart audits with no identifiable patient data and knowledge tests will be sent from the clinic sites by mail or fax to the sponsor’s office at the department of Clinical Programs and Quality of Care at the American College of Physicians headquarters building in Philadelphia, Pennsylvania.
